End of the contract, what paperwork needs to be taken care of?

You and your childcare provider should both sign the contract in order to show that it has been terminated and on which date, just so this is clear to any outside parties.

If you are the recipient of a provincial subsidy which is being paid directly to your childcare professional you need to notify the appropriate ministry in your province that you are terminating care with the caregiver.

What documents should she provide?

 Do not get caught without receipts from your home caregiver! You will most certainly need them to apply for tax credits and other government childcare subsidies.

What do you need to provide in the way of paperwork.

Since the home childcare provider is technically considered self-employed you will not need to fill out a Record of Employment (RoE) or a T4 or other tax related documents.

If you've enjoyed the care provided by your caregiver, why not write her a reference? Surely if she plans to continue in this line of work she will appreciate it and it will be useful to her. Of course, if the relationship ended on bad terms or you would be wary to recommend her; it's not required!
